LIABILITY WAIVER – PLEASE READ BEFORE PROCEDING, SUBMISSION OF REGISTRATION CONSTITUES AGREEMENT TO THE LIABILITY WAIVER: By submitting this registration form I hereby release and hold harmless on behalf of myself, my child and/or representatives and any other persons whom I am registering for, DeWitt Township, Granger Meadows Park, Granger Recycling, Sparrow, Sparrow Foundation, Sparrow Hospice Services, the event sponsors and organizers from liability for inquiries or damages which I or my child may sustain while participating in this activity, even if the injuries are caused by the sole negligence of DeWitt Township, Granger, event sponsors or the event organizers. I understand that I am responsible for the medical coverage for my child and myself. I also authorize Sparrow to make, reproduce, copy, exhibit, publish, distribute, and otherwise use for publicity, fund raising, advertising, or any other lawful purpose photographs, videotapes and sound recordings taken of me or my child.
